Chords for Italy's Song to Germany

Just as title says,
I'm looking for the chords for Italy's Song to Germany.
Tabs would be prefferred; I can play a little guitar, but can't transpose music to save my life.
I'll be cosplaying as Chibitalia and singing/playing this song at my Artist Alley table for donations to Red Cross Japan.

Help in finding or providing chords would make me a VERY happy person.

Haha I enjoyed the song that the chords didn't stand out, had to repeat many times.. Hope this helps:
E, C#m7, C#m on G#, C#dim on G,
B7 on F#, C#m7 on G#, C#dim on G, B7 on F#, followed by the notes E F# G# with the chord E on G#,
now I'll just say the notes (I just gave up on the chords...) D#F#G#CF#G#, C#EG#BC#G#, A#C#EG#C#E, (now are the chords): E on B, C#dim, E on B, C#m, E

*not sure if I could even hear the bass note, Italy sure sings loudly..
**Aiya hearing it repeatedly makes my head hurt (not the ear, the music's nice!) [was playing loudly on the headphones to hear the chords]I shall just post this half-done song. Hope you'll know what chords I'm trying to say! Maybe others can help you aru!

I REALLY, really apologize. I'm not sure how to read guitar chords like how you're explaining them to me.
I'm only used to reading tab sheets since I have no instruction in music of any kind...and have no one in my family that does either.
Is there any way you could possibly put this on a tab sheet?
