They really are brothers? I thought Italy just called him nii-chan because Spain is older than him.

that was my first thought before, but as I re-read the chibitalia arc, there is no mentioning of France, Spain and Italy as brothers. They only live together. The only brothers in there is N.Italy and S.Italy. Even Holy Roman Empire, Austria and Hungary lives with them. Italy just addressed them with nii-san because they are older, not because they are brothers.
