It looks like you're supposed to send that postcard from your copy of vol. 4 to the secretary's office and they'll email you the info on how to pay (it'll cost 1000 yen to get the CD). There's a special warning for people using a domain-specific spam blocker in your email; be sure to enable mail from "@y-logi.com" because if they're unable to deliver that message, you'll no longer be eligable to receive the CD.

There's an explanation about how to pay under "お支払方法の説明はコチラ" (I'm assuming it's a link for you?). Since they've set the 1000 yen charge, which I imagine is to cover shipping and handling costs, I doubt anyone outside Japan is eligable for this offer since it costs more to ship internationally. It doesn't specifically say that; that's just my guess.
