AHAHAHA, ROME YOU FREAKING VOUYER, COMING BACK FROM THE DEAD TO HAUNT ITALY.

That entire series is pretty fun (I totally love Rome), but actually what it is is that Italy woke up before Germany; The perspective is a bit weird, but basically, Germany is still sleeping, and the panel is from right behind his head, looking at Italy waking up on the other side.
STILL AMUSED THAT ITALY IS SLEEPING NAKED IN GERMANY'S BED

Ohh Grandpa Rome! I hope this get's translated soon it looks so interesting. That panel is very suggestive though, if we didn't know Italy better I'm certain that pose could be miss construed.
